School(s):
Medicine Pharmacy
Semester(s):
Fall Spring/Summer Winter
Type(s):
Clinical/Community Experience

Designed to explore the feasibility and impact of incorporating intentional interprofessional team pre-rounds (“IPT pre-rounds”) into inpatient acute care rotations. A pharmacy student will form a team with a med student. The team will meet with the pharmacy faculty once a week to discuss their mutual patient case. During the IPT pre-rounds, the team will co-create a patient-centered treatment plan for their mutual patient through shared decision-making. The team will be encouraged to engage patients/caregivers and consult other members of the patient care team if their skills and knowledge are needed to optimize their patient care plan and enhance the outcomes. The final IPT patient care plan can be presented at the next patient care team rounds or documented as a draft patient care team progress note. We anticipate that this in
